Measuring 12 x 8 inches, Clarke is pictured in a full-length pose, with his name and a brief description of his role with the Pirates underneath. An "Index Co. 1906" copyright is present as well. The supplement is affixed to a larger 11.75 x 15.25 calendar planner page. The black-and-white image of Clarke is crisp and clean, with a small spot of paper abrasion near his foot.
